They are adult sized by waist. I'm a 32 inch waist and use a medium size in these pants. They are returnable. You can try a few sizes and send the others that don't fit back at no cost with free returns with Amazon.

I think they may refer to the hip size. I bought the size I usually buy in underwear or shorts - large. I really didn't pay attention to the numbers!
