I don't recall if there were any. There are 2 'key-hole' holes in back to affix to screws in the wall. I recommend trying to affix to a stud. Currently I have just leaning against the wall with abut 1/4" paper under the right and left bottoms just to angle back until I can affix more permanently.

Question 3

What is the wood coated with? I need to confirm this to ensure I buy the right kind of paint.

First, let me say I adore my fireplace. It appears to be coated in a soft gesso-like white colored material. Very delicate.  When it arrived there was a little damage to the box and consequently the fireplace had a little part that was damaged.  When I tried to brush on the same color with acrylic paint the white substance just fell off like salt.  When I tried to brush some of the surface off in order to get some contrast highlights, I was hoping for just a lighter shad of the top color. But instead the white gesso-like substance was revealed, so I did not proceed.  I'd use an acrylic paint.  Oil-based, in my opinion, would 'drag' and/or 'soften' the existing faux finish. I tried to paste a photo of the area, but doesn't look like it came through.  Again, this is an amazing, beautiful addition to my home!

How wide is the top of the mantel from front to back not side to side?

It probably varies from 6-9 inches wide on the top, woth room to place vases and sculptures. There's a slight curvature to the edges. The top overhangs the vertical elements by about 3-4", then the decorative base is a couple of inches larger than the columns. It's sturdy, standing on its own. I don't have pets or young children so I'm not worried about it tipping over, but there are reinforced keyhole mounts on the back to secure it. If I had children, I'd screw my own larger L brackets on the sides and into the wall to make it tip-proof.
