Has anyone actually installed this to a Wilcox Crittenden head mate unit??

I installed this unit on a Jabsco toilet, but the fit seems to be universal and the units appear to be the same. Hope that helps...

Is the back flow preventer simply a joker valve and do you have a choice of flushing with or without water

It's an inferior design joker valve. It's merely a joker "flap". It's a non standard design, can you get parts anywhere? The combination of the intake fluid with the evacuation fluid on the same pump sounds like a simple solution, but you lose control of the ending water level in the bowl.

Will this work properly with a pressurized fresh water system? my system has a separate valve to fill the toilet.

No, I don't think it holds the pressure, this macerator pulls salt water into the bowl and at the same time push it to the holding tank.
