Am I supposed to apply the ice directly to my forearm or do I want to have something between the ice and skin?

You rub the ice directly on your skin. Do it for 5 to 6 minutes. It will numb the area for about 4 hours, and decrease the inflammation. 2 to 3 times a day is best. It will be cold, then hurt a bit, then become numb. Really not too bad.

Are they dishwasher safe?

I did not receive the cup as indicated and had to return. It is not a big item to even consider putting in dishwasher. It is plastic. So, putting in upper rack of dishwasher should be ok.

Wouldn't this turn into a dripping mess?

I use a towel so it catches any drips but this is a great cup and I use it regularly.

how do you freeze it?

It's a two part cup, the "Ring" handle fits like a top into a "cup". you just sit the filled cup in the freezer.
