how do you know what size to get?

This is from the product description/specifications: Specifications: Available in several sizes:. X-small: women's 3.5. Small: women's 5.5-8. Medium: men's 6-8, women's 8.5-10. Large: men's 8.5-11, women's 10.5-13. X-large: men's 1.5-14, women's 13.5 plus. For some reason, it only showed up when I selected large - but I've learned from other things that sometimes different information shows up under different options. I wear a size 10 shoe, but I ordered the large. Because the bulky cast pushed my foot forward, it was the right decision. Toes came right to the front edge.

Can this be worn instead of "the shoe balancer" to level the opposite foot while wearing a fracture/walking boot. what is the height ?

No. You could wear it INSTEAD of the boot you have though. What you're calling a fracture/walking boot is medically called a CAM boot, for Controlled Ankle Motion. Wearing two will likely cause you to fall and it would be extremely difficult to walk.

is there padding or cushion on the bottom

This boot has some cushioning, but not a lot.

There is no men's or women's. It's a cast. It's unisex. Each size fits a range of men's and women's shoe sizes and the vast majority of women will need medium and the vast majority of men will need large.
