What's the difference between this and the Shoulder Dolly?

Not sure what a shoulder dolly is, but we were able to easily move a huge heavy couch and gun safe with no problems.

Need to move a 22' dress or. Can I buy a strap for something this large?

We don't have one that long, but buy two 100 foot long 2 inch wide cotton straps (sold on Amazon) and then watch YouTube videos about using monster straps to move extra long furniture.

The Shoulder Dolly can help you lift anything you can safely balance on the wide strap, including sofas, mattresses, chairs, dressers, stoves, and dryers. The lifting is with you back, leaving your hands free to help balance the item. Even rocks could be moved if the strap could be secured underneath them, perhaps via a box or tray.

Is this adjustable? Can the middle strap be shortened?

Yes you adjust the middle strap on either end of the harness by pulling more through the orange piece.
