I recieved my o cedar mop bucket set but did not get the 3 xtra mop head refills , has this happened to any one else ?

Yes, I had the same problem. I called right away and they shipped the mop heads right away.

are mop heads washable?

Yes;just pop them in the washer with some grubby cleaning rags. I let mine air dry.
