Does the protractor head have a spirit level?

Yes there is a level on the protractor and square head.

Are the graduations scribed or painted?

Mine I bought years ago and seems like new. Scribed

Does it have measurements on both sides and how thick is the blade ?

Yes it has measurements on both sides one side has 1" to 24" in 32nds and 1" to 24" starting from the other end this edge in 64ths. The other side layout the same one egde in quarter of an inch the other edge is 16ths. The blade is 24" x 1" x .005/12 this last measurement came from the packaging that came with the ruler. I measure it at 3/16".

what is the head made of?

It appears to be hardened steel and very accurate
