his H&P medical planner notebook has helped me tremendously during clinical rotations. Having a dedicated place to organize histories, physical exams, and assessments has made such a huge difference in how confident and efficient I feel on the wards. It keeps everything structured and easy to reference, especially when days get busy and fast-paced. This H&P notebook helps organize everything you need to ask about/find out about patients.

This was great for my clinical rotations. I was using a plain notebook but really wanted to have some organization with my H & P. The only thing that was kind of disappointing is the size of the notebook. It’s a little too small for my big writing, however, it fits conveniently in your white coat, which is nice.

I originally got this for my boyfriend when he started 3rd year medical school rotations and he absolutely loved it. He said it is great especially for IM. It helped him organize his thoughts and notes and not forget to ask anything. He then got one for me and I agree! This is super helpful especially for medical students and interns, as they may need a little more guidance. It could also be helpful for senior residents and attendings, but less necessary since they are more experienced and may not need that level of guidance.

I just completed my first semester of clinicals with my PMHNP program and this H&P notebook was perfect for taking notes throughout the appointments. It was small enough and it did not distract from the patients experience.

As a 3rd year medical student who recently began clinical rotations, this notebook is great for keeping me on track with the H&P and helping me to organize my findings and notes for better recall later. I especially like the review of systems as it seems to match the level of detail of what is expected of me (I'm currently on internal medicine)- the ROS is not too deep in the weeds but has ample coverage of every key system. Just be sure to write small! I'm aware that making this notebook any bigger would limit its portability, but I wish they'd give just a little more space for the blood pressure reading, patient name, MRN, and chief complaint as I struggle to fit my letters and numbers into the spaces provided. Also, I didn't think the differential diagnosis spaces placed below the CC helped much; most of the patients I'm currently seeing have more than a few comorbidities and in IM they prefer to have the DDx listed by organ system (or problem list). I can't speak for most other specialties though. All in all, very happy with my purchase and well worth the money! Saves a lot of time and frustration on the wards, and the organization will make your progress note writing faster if you typically suffer from poor note-writing like me.

I bought this notebook before starting my clinical rotations. It is helpful but has very small boxes to fill in for some of the history information. It also runs out very quickly and is slightly overpriced in my opinion. I bought it once to help take histories and now don't use it at all. Will not be buying another.

The book was covered in sand and then when I opened it up it was clear why… water damage 🤦🏼‍♀️
