Is the lunch bad insulated?

No it is not. My ice pack melted in less then 4 hours and all my food was warm.

Does the diving sauce dish fit into the spoon and fork compartment?

Not that I have noticed, this set is smaller than what it appears in the picture.

does the lunch box and soup container fit into lunch bag comfortably?

Yes it does fit. You can also add a water bottle, orange, apple whatever that is small. The bag does have room to expand.

Is there a divider between the top and middle compartments?

No, there is no cover on the middle box. There is one for the bottom box.
