Can someone tell me how far apart the screw holes are and if the screws are included with the package?

I don't think screws were included (though I've forgotten what exactly was the packaging), since what you'd need would vary depending upon what chair or surface the screws were going into. From my measurement from the center of one screw hole to the center of the other it's nine centimeters. Hope that's helpful.

What size bolt do you use for steelcase leapchair v2? i saw 1/2 inch bolt with nut, but what about length? thanks.

Sir, It will depend on what your chair back is constructed of. If it is metal 1/4 inch would be sufficient. If plastic as mine, 5/16 bolt may do with nut and washer; get one long enough to put a large diameter washer for support to help keep from bending. I had to drill a 5/16th hole through the top, same as bolt to keep play out. All will vary with material and strength of chair back. Don't over do it as there should not be much pressure on it. No clear answer due to too many variations. I would not recommend tapered screws into plastic good luck. GF

I don't know how the Back of Aeron chair looks like. Lorell Head rest has two holes which are approx 3.5 inch apart. If your chair back has any open holes which are 3.5 inches apart, then you can fix this head rest.
