Does this include the lab coat or just the wig and mustache?

Just the wig and moustache. We had to borrow the lab coat from a relative.

is the mustache single use only?

I used double sided tape and could make it a few hours before the tape wore out, I used it for 5 days for our church VBS, so the mustache can last. You'll just have to use some costume glue or tape to make sure it stays stuck on.
