does this part come with plastic control arm?

No, it's the actuator. There is a keyed shaft and a wiring harness, but no control arm, etc.

Does the part come in a small cardboard box with a dorman sticker and barcode?

If I remember correctly it came in a small box with wrapped in bubble wrap with a dorman sticker.

I have a 2013 impala that needs 2 door actuators 604-108 replace but Amazon says (this part does not fit your 2013 Chevrolet Impala) ?

I believe it will fit - I looked at some of the online auto parts stores and they say it does! Suggest you do an online search for "dorman 604-108" and double-check with others besides Amazon. 2013 Impala has several different models, (LS, LTZ, etc.) but they're all basically the same car with engine, interior, and or other upgrades.
