No, it doesn't. It will cut it unevenly so one half is markedly larger even when lined up perfectly with a scored pill. It cuts the top evenly but the inside goes crooked.

There is a tab on the left front and one the right. The left is part of the top and the right is bottom, I put my thumb and fore finger together thumb under left tab fore finger over right tab and rotate clockwise. Or put the ball of your tumble over both and rotate clockwise.

This splits a tiny lasix pill I give to my dog! Make sure you are positioning the pill in the pointed end of the triangular shaped compartment that houses the blade. Keep that end lower to hold the pill in place when you cut it.
