Will it fix a teenager

Sure. There are 3 sizes, from toddler to youth. Normally size M or L would fit a teenager. Please choose the size according to the head circumference. Have a nice day.

Are the inside adjustable parts around the head plastic or fabric? My grandsons currently have the type with plastic and won’t wear them.

Plastic, but helmet comes with two (thick and thin) cushions attachable with velcro.

My son has a head that is slightly bigger than 20 inches so will the medium be able to be small enough to fit so we can get one that will last longer?

I have 2 small helmets and one of them adjusts to a smaller size than the other. Personally I would try medium because with the extra padding and the adjustable back you should be able to get to where you need to be
